The HTML template gets included as an attachment into your email with the results if within your Survey Settings you selected the option " Send GPS, Picture & Voice files by e-mail "
The Customized HTML template will basically look exactly as the paper form you will be replacing and it will position every question and answer into that exact location on the eForm
